1. Who are you?

By submitting this form, I acknowledge the following:

  • All hardware and software listed are the property of EFM Interests, DBA Re-Bath and are issued to me solely for business use.
  • I am responsible for reporting a complete and accurate inventory of all company-owned devices, equipment, and accounts currently in my possession.
  • I understand that knowingly omitting or failing to disclose company property in my possession may be treated as misappropriation or theft of company assets and may result in disciplinary action up to and including termination and/or legal action.
  • I agree to notify IT of any lost, damaged, or stolen equipment as soon as possible.
  • I understand that all company property must be returned upon request or upon separation from the company.
